About This Webinar for Physicians and Healthcare Professionals
A case study of a 66-year-old patient provides the backdrop for two diagnostic scenarios—sepsis and post-sepsis syndrome, both of which are leading contributors to in-hospital mortality and complex post-acute care.
Attendees will appreciate the impact of sepsis and post-sepsis syndrome to the US healthcare system, describe hospice eligibility for patients experiencing sepsis through hospitalization and post-acute, understand indicators of poor prognosis in sepsis and post-sepsis syndrome, and discover how hospice can help support patients with sepsis and post-sepsis syndrome. Also, learn a clinician's mnemonic for sensitive discussions and how to address common misconceptions through an example of an effective goals-of-care conversation.
This webinar is offered for both CE and CME credit:
- CE credit is available to nurses (RN, LPN, LVN; in California Registered Nurses only), social workers and certified case managers, as well as licensed nursing home administrators in the states of Florida and Illinois.
- CME credit is available to physicians (MD, DO), nurse practitioners (NP) and physician assistants (PA).

Nurses and Case Managers
To earn CE credit, attendees must view the webinar in its entirety, complete an online, post-event evaluation and download their own CE certificate. Participants must register and sign in to the live webinar using the same email address.
This activity is approved for one continuing education credit hour for nurses (RN, LPN, LVN, ARNP; in California, Registered Nurses only), social workers and certified case managers. Licensed nursing home administrators in the State of Florida and Illinois may also receive one hour of continuing education credit. Attendance for the entire online webinar, as well as completion of a brief online evaluation, is required. Certifications are made available after completion.
Physicians
Satisfactory Completion - Learners must complete an evaluation form to receive a certificate of completion. You must participate in the entire activity as partial credit is not available. If you are seeking continuing education credit for a specialty not listed below, it is your responsibility to contact your licensing/certification board to determine course eligibility for your licensing/certification requirement.

Lauren Loftis, MD
Regional Medical Director - Northern Florida and Alabama, VITAS Healthcare
A family medicine physician specializing in end-of-life care, Dr. Lauren Loftis now serves as the regional medical director for VITAS in Northern Florida and Alabama. In this role, she leads the medical care of hospice patients and their families throughout Citrus County and the Nature Coast, Pensacola, Panama City, Jacksonville, Tallahassee, Mobile-Daphne and Dothan.
Dr. Loftis joined VITAS in 2022 as medical director in Brevard County where she delivered comprehensive pediatric palliative services for children facing serious illness and interfaced with hospitals, skilled nursing facilities and other facilities to consult and develop services for hospice and palliative care.
Dr. Loftis is board certified by the American Academy of Hospice and Palliative Medicine (AAHPM) and the American Board of Family Medicine (ABFM). She also received the Hospice Medical Director Board Certification (HMDC) and is a fellow with AAHPM.
She completed her medical degree at the University of Texas Southwestern Medical School in Dallas, Texas, and holds a bachelor’s degree in biomedical science from Texas A&M University in College Station. In addition to noteworthy awards honoring her dedication to hospice, Dr. Loftis has also contributed to scientific publications, particularly on the topic of childhood cancer survivors.
